IHE Getting Images From The Department to The Enterprise to The Cloud

XDS-Cross Enterprise enabling image and report sharing
-Requires MPI and flexible gateways
-IHE website is a great resource for interoperability Www.ihe.net
-Viewing images by patient location key for enterprise/cloud users
-Browser based-EMR linked-ability to launch more sophisticated viewers (3D, specialty viewers, etc)
-Image life cycle management- how long do you keep them local? On the Cloud?
-A lot of effort goes into integrating the ologies -not every imaging system follows standard workflow- jpeg, tiff, bitmap, PDF , etc….
-Many methods of peer to peer and social network sharing available now but they don’t work for Healthcare Privacy
-Who pays for exchange- Sender or receiver or Patient?
-With PHR patient controls who sees images or reports with secure code and password
-Dell and Lifeimage both offer PHRs with more companies coming online.
-Mount Sinai, UCSF, U of Maryland, and Mayo are using RSNA image share- Texas Children’s joining soon. Funded by NIH

Informatics and Meaningful Use
Stage 1 organize data
Stage 2 improve clinical processes
Stage 3 improve clinical outcomes
-Must use CEHRT that meets your needs
-Imaging providers must be able to provide image viewing to eligible providers via their CEHRT
-Patient Encounter is only Physical patient contact not remote reading.
-RIS with built in logic will aid in Clinical Decision Support requirement
-Must have organization MU program
